New Patients
We are excited to welcome you to Premier Fertility Center. Our team is dedicated to helping those who desire to build their family. Let us make a few comments about your care at our fertility clinic.
1. All questions are good questions and worthy of an answer. Please feel free to discuss any issues that you may have with us.
2. While we do not post credentials in our waiting area, Dr. Deaton is a Board Certified Reproductive Endocrinologist, Infertility Specialist and our labs are nationally certified. The nurses and lab personnel also keep up with their certification requirements. We will be happy to share this information with you.
3. New patient appointments at our office tend to last 30-45 minutes and spouses are welcome. At your first visit we will discuss your needs and develop a treatment plan. There may also be an opportunity for certain fertility tests to be done at this visit. Please bring records and a completed new patient form with you or have them sent to the office prior to your appointment.
4. Infertility treatments often involve frequent visits to our office for ultrasounds and inseminations(IUI). You may not see Dr. Deaton at every visit but he reviews all information concerning your care with the team on a daily basis. If you need to see Dr. Deaton, please notify either the nurses or our receptionist.
5. Infertility treatment can be costly. Please refer to the enclosed financial information sheet.
6. We understand the sensitive nature of your infertility and make every effort to protect your privacy. If you have specific concerns in this area, please let us know.
Finally, since we dedicate our practice to infertility, we fully understand that you are in the middle of a very difficult trial. While the focus is often on the physical nature of infertility, we do not want to ignore the emotional or spiritual aspects of your struggle. We also understand that some marriages are stressed during this time.
In addition to expertise within our fertility treatment team, we also keep up with local resources such as support groups, adoption agencies, and counseling services. Our goal is to assist you through the trials of infertility and the decision making process regarding your future. While a pregnancy is an obvious goal of ours, we understand that a success can take on many forms. We are also delighted to help couples along other avenues such as adoption. Please keep us informed of any needs you may have. We all look forward to working with you!

Some of these forms are for "our patients only" and can only be downloaded by submitting the appropriate username and password.
